What is face reduction (jaw reduction)?
Face reduction (jaw reduction) by surgery, mainly jaw contouring surgery, which can also be combined with sliding genioplasty surgery or chin implant (discussed in a separate section), extracting cheek fat to shape a slim V-Line face.
What we need to know when proceeding with face reduction surgery
- Jawbone is a bone lying beneath the ear. A large jawbone makes the chin corner look thick, while making the facial features appear unnatural and coarse.
- These days, jawline contour surgery is a common surgery in shaping the oval, V-Line face for Asian women.
- The bony protrusion in the angle of the jaw on both sides is a common characteristic among East Asians. Even though a square, angular face isn’t considered a disease or deformity, it can greatly affect the aesthetic and self-confidence, especially for women.

The process of face reduction surgery
- The doctor will conduct a general examination of the features on the client’s face, and comment on the imperfections that need to be corrected.
- General health check-up and examinations and take 3D X-ray images in order to evaluate the detailed jawbone structure and accurately before proceeding with the surgery.
- The patient undergoes the surgery under endotracheal anesthesia.
- Using an incision completely inside the mouth in the lower jaw area. The incision is hidden inside the mouth, making it very aesthetic.
- Gently dissect the tissue to the position of the lower jawline and the jaw bone that needs surgery.
- Performing the bone cutting with a specialized ultrasonic bone cutter in maxillofacial surgery, so it is painless.
- Currently, there are many methods for mandible angle osteotomy tailored to specific clinical cases, such as: curved mandible angle osteotomy, V-Line Osteotomy, vertical split osteotomy and the outer mandibular osteotomy combined with chin reduction and buccal fat removal. Sliding genioplasty surgery
- Sliding geniplasty surgery is usually performed in combination to shape the perfect V-line face. The procedure is the incision inside the oral cavity at the lower jaw (same incision with jawline contour), the surgeon will remove the bone at the middle of the chin to reduce the chin. The size of the central bone depends on the thickness of the outer bone at the angle of the jaw and the horizontal wing. The bone is secured with a structure of titanium tube system.
- Sliding geniplasty surgery is usually performed in combination to shape the perfect V-line face. The procedure is the incision inside the oral cavity at the lower jaw (same incision with jawline contour), the surgeon will remove the bone at the middle of the chin to reduce the chin. The size of the central bone depends on the thickness of the outer bone at the angle of the jaw and the horizontal wing. The bone is secured with a structure of titanium tube system.
- Extract the buccal fat pad: There are 2 layers of fat on the face, 1 thin layer beneath the skin, and another layer lies deep inside the cheek called buccal pad. When we want to eliminate the fat layer beneath the skin, we suction the fat. But, the buccal fat pad must be removed from inside the oral cavity to shape a slim face.
- The incision inside will be sutured using absorbable thread (without removing).
- Place a tube to reduce pain and swellings after undergoing V-Line face contouring surgery.
- Patients are discharged on the next day and have their drainage tube removed. Use antibiotics,anti-inflammatory and pain relievers from 7 to 14 days after the surgery to avoid infections and reduce face swellings.
Post-operative care
- Compressing ice frequently in 48 to 72 hours after the surgery help reduce swelling.
- Use specialized jaw compressing bandages continuously for at least the first 1 to 2 weeks after the surgery helps stop bleeding and reduce swelling.
- You should prioritize liquid and soft food (milk, porridge, soup) in the first days after the surgery.
- In the first week, limit tooth brushing to avoid impact on the incision. You should rinse your mouth every morning-evening,, and after eating with Kin mouthwash.
- After a week, you can resume brushing your teeth with a small, soft-bristled toothbrush.
- Avoid strenuous exercising, excessive mouth muscle activity, chewing hard food, and impact that can damage the wound.
- Avoid talking much or chewing on hard food in the first week to reduce haw bone activity.
- Take medicines and have the following check-ups according to the doctor’s appointment.
- Do not touch, stretch the lips or have hard impact onto the incision wound.
- Apply Vaseline onto the lips to minimize dry or peeling lips.
- After the surgery, if there are any abnormal complications such as: bleeding, excessive swelling, high fever… immediately contact and visit the doctor.
Is face reduction - jaw reduction surgery painful?
Face reduction surgery doesn’t hurt, only has light pain in the first 24 hours, and you can use pain relievers. However, haw reduction surgery can experience swellings that last from 2 to 4 weeks.
In cases where the client has a coarse face due to simultaneously having high cheekbones and large jaw angle, jaw reduction can be performed in combination with cheekbone reduction.
